Getting Started
 
All physical therapy sessions require a referral, no matter your insurance, as it is a state law. Referrals to INTEGRATIVE can be made by your physician or specialist, nurse practitioner, osteopath, chiropractor, dentist or podiatrist. Referrals may either be faxed to INTEGRATIVE at (757) 216-4152 or brought in at the time of your first appointment.
 
As every patient's insurance varies, we ask that you familiarize yourself with your insurance's physical therapy benefits, including your co-pay, so that we may begin your care.
 
 
Making an Appointment
 
To make an appointment, please call our office and we will be glad to assist you in scheduling. The availability of appointment times varies depending on the service you are interested in. Appointment times generally last from 1 hour to 1½ hours. 
 
Please note that when making an appointment for physical therapy, you will be asked to complete a detailed health history as well as the specifics related to your current condition; this information helps us to better understand your individual needs and create a treatment plan.  This health history can be found on our website under the link Initial Paperwork. Simply print it and bring the completed copy to your first appointment.  Upon request, we can also fax or e-mail this paperwork to you prior to your visit.
 
Should you need to cancel or reschedule an appointment, we require 24 hours notice, as we often have a full cancellation list and would like give another patient the opportunity to utilize this appointment time. Missed appointments or appointments cancelled without 24 hours notice are subject to a Missed Appointment Fee.
 
What to Expect on your First Visit 
 
We encourage you to come 15-20 minutes early for your initial visit to settle in comfortably and allow yourself plenty of time for filling out paperwork. If you have not already done so, you will fill out a detailed health history as well as the specifics related to your current condition; this information helps us to better understand your individual needs and create a treatment plan. 
 
Your therapist will then sit down with you to discuss your physical findings and treatment plan and, time permitting, begin treatment.
 
Be prepared to pay your co-pay or self-pay at the time of treatment. We accept payment in the form of cash or check.
